Polish the Layout
It’s easy enough to slap a bunch of form elements on a page, but you’ll get more responses and better ones if you design the form so that it is easy to read and use. At the very least, you should align the elements neatly and label them so that the respondent can see which elements go together, and so that they can figure out what information is actually being requested. Use a Grid
