How do you build a just culture in your organization?

A large number of organizations I have met believe (or perhaps just hope) that it is a matter of applying a simple formula. That all they need to do, for instance, is distinguish between human error, at-risk behavior, and recklessness—categories that imply increasing degrees of willfulness and disregard.1 I can understand that the idea of a formula, and such a three-step model of badness, is vaguely reassuring. It is like matching symptoms to a disease.
