Chapter Seven ACTION GUIDE

As noted in Chapter Seven, acute care has many handoffs and transitions in the patient's course of care. In these white spaces the patient is at risk for transfer of incomplete, inaccurate, or absent information specific to the patient's unique needs and plan of care. Transition from ambulatory care to emergency medical systems such as an ambulance with trained staff and then to emergency departments requires exquisite communication and advance planning of the handoff processes.
