Transformation Steps 6 and 7:Involve people, benchmark possible solutions, and identify exemplars: the Design phase

Now that you’ve done your detective work and the challenges you face are glaringly obvious—you are ready to begin the Design phase. This phase has two purposes:

1.  Develop a strategic plan—develop a one year plan for health management including the goals, objectives, tasks, accountabilities, timelines, budget, communications and evaluation plans for the effort. In other words, decide what you have going to do to improve individual and organizational health and who, what, when, where, and how you are going to get it done. One important component—it helps if the plan is designed so that it will be approved and funded!
