OPTIONS: LETTER FORMAT OR ACTION FORM

In documenting the disciplinary meeting, you can use either a letter format or a disciplinary action form. The letter relies on you for its structure and content; you write it “from scratch,” following an outline for the body of the letter. The action form offers you, to some degree, a ready-made structure; it provides basic check-off information and a fill-in area for detailing the disciplinary situation (for a simplified action form, see next page). The fill-in area demands your clear and structured input, though, just as the body of the letter does. The content will be one and the same.
