Conclusions

Learning from experience is the most basic of human activities, but we have explored in this book how difficult it can be to scale up from learning as an individual human, to learning as a complex organisation of many humans.

We have learned that lesson identification is something that needs to become routine and proactive. When discussing the various methods of lesson identification, we found that many or most rely on effective questioning, and on identifying the root causes behind events. This was further illustrated in Chapter 13 by Johnny Martin.
