These are the credits, which I’ve sorted into categories, even though many people I list defy categorization.

My wife Joyce, without whose faith and gentle pressure (over more than twenty years), this book would not have been written. Also, our son Jeffrey, who compiled the bibliography and who has been patient with my nonparental preoccupations.
