Instructions: Transfer your scores for each item in the Corresponding Statements column onto the appropriate line. Then sum the items for each sub-dimension, and divide by the number in the Average column. Finally, check the box (strength, area of potential, area of opportunity) that corresponds to your score for each sub-dimension.
