Writing the Disciplinary Report

When writing the disciplinary report, state the nature of the discipline being imposed and the reason(s) for it. There is no need to recapitulate all of the facts of the offense if they are properly documented elsewhere, but you should at a minimum state the ultimate facts in a brief statement. For example, you could write one of the following things:

  • You have been absent (or tardy) on seven occasions in the past sixty days: May 8, 10, 15, and 21; June 4, 5, and 12.

  • You were observed clocking out for ______________ [name the employee].

  • You started a fight with ______________ [name the employee], who had done nothing to provoke this kind of violence.

  • A search of your locker resulted in the discovery of a half empty ...
