As with just about all books ever written, the author did not complete his work alone. This book is no exception.

First and foremost, I would like to thank my wife Susan for all she did to help bring this work to fruition. Although it might be trite to thank a spouse who did none of the research nor wrote any of the words, Susan gave me a few things that were more valuable. She took on some of my responsibilities around the house and with the children to give me time to work in the evenings.
