Answers

A1: You should use telnet only when ssh connection attempts are not answered by the remote system. When this is the case, you should consider asking the administrator of the remote system to add support for ssh.
A2: In each case, the former command (cd, pwd, ls) operates on the local system in an ftp session, and the latter command (lcd, !pwd, !ls) operates on the remote system.
