Short Form Applications
The Copyright Office offers short forms for certain literary works, works of the performing arts, and works of the visual arts. These short forms are one page long and take only a few minutes to complete. Generally, the short forms may be used in the following circumstances:
The author is the only author of the work, is living, and is the sole owner of the copyright.
The work is completely new (rather than a derivative work or revised edition of an earlier work).
The work is not one made for hire (thus, the work cannot be one prepared by an employee within the scope of employment or a specially commissioned work).
If these requirements are satisfied, the short form application may be used.
