Olympic Pipeline Company Gasoline Pipeline Rupture

Figure 15.1 shows the result of the Olympic Pipeline Company pipe rupture in the Bellingham, Washington, cyber incident. The investigation concluded it was not caused by an intentional act. Because of the detailed evaluation by U.S. National Transportation Safety Board (NTSB), this is arguably the most documented ICS cyber incident. According to the NTSB Final Report, the supervisory control and data acquisition (SCADA) system was the proximate cause of the event. Because of the availability of that information, the following detailed postevent analysis was performed.[1]
