Recall the fictional case example presented at the beginning of the book (the fictional one-hour, instructor-led workshop on how to plan and hold a meeting). The risk for the project was a 2.4. The workshop requirements are the riskiest part of a project because they define what the content of the workshop will be.
A list of anticipated risk factors is part of a risk-scenario document. The project manager must remain vigilant in looking for signs that these risk factors are actually coming to fruition. Some of the signs that might indicate trouble along this front are
new customers with different favorite approaches;
a “we must build it here” attitude from the customer, which prohibits research or external help;
a “we must get it outside” ...
