Willful violations (A) are, in addition to having the potential for serious harm or injury, done intentionally or with "plain indifference" to the requirements. Repeat violations (C) are violations for which an employer has been previously cited and have recurred. De-minimus violations (D) are unlikely to cause serious harm or injury.
