new_padDEPTHnew_padDEST,DEPTH
Create a new lexical scope pad. If a destination PMC is provided,
store the pad in the PMC, otherwise push it onto the lexical scope
stack. DEPTH specifies the static nesting
depth for the pad (lower static depths are copied from the current
static nesting).
