new_padDEPTHnew_padDEST,DEPTH
Create a new lexical scratchpad. If a destination PMC is provided,
store the pad in the PMC. Otherwise, push it onto the pad stack.
DEPTH specifies the static nesting depth
for the pad (lower static depths are copied from the current static
nesting).
