Appendix B. The Denison Survey: Questions

Respondents to the following survey questions are asked to assess how strongly they agree with each of the following statements with reference to their own organization (on a scale of 1 to 5, where 1 is strongly disagree and 5 is strongly agree). After assessing their organization for each individual item (question), the respondents then develop an overall assessment for each index based on their answers to the five items for that index.
