A Google search for Requirements Specification Template yields over 10,000 hits. After looking through a fair number of them (more than 10, less than 10,000), I do not think you need yet another template from me. Instead, I will give you an annotated outline that covers the most important elements of a Requirements Specification. You can turn this into a template, or you can look at some of the ones available on line and pick one that matches your exact needs.
