Solutions to eliminating process waste

Most of what has been described earlier happens because it's hard to take a big picture view on processes as we evolve as a business. We fall back into the "we have always done it this way" mode and try to adapt best practices and apply them to our context. We set up story boards, have stand ups, estimate stories, measure velocity, and reward bug finders. It has worked for us in the past, or everyone says it has worked, so we believe that it can't hurt us. This seems natural.
