Location in the Survey

So where should such questions be located in the survey? Generally, they are positioned as the first items, but, on occasion, they are positioned as the last item. By placing them first, it immediately communicates to the participants what identifying information is required. If they are the final items, the instructions should mention that such information will be requested at the conclusion of the survey but that it will be used only for reporting by employee groups.
