Prior to 9/11, the military hadn’t been in a sustained engagement since Vietnam. The decades-long Cold War gave rise to the military bureaucracy. Decision making went from the “boots on the ground” to the top of the chain of command. With nuclear consequences, all decisions were so strategic that they had to be pushed up.
