There is more. The fundamental model for engineering software has been built on a linear sequential model with a disconnected view of the customer. Even the fundamental tooling infrastructure, configuration management, defect tracking, call tracking, and testing harnesses were all developed with the idea that you could not have complete knowledge of the software. Most defect tracking systems for software are little more than online sticky-note systems that use unstructured text and email to compose a picture of what is going on in a customer's computer system.
