Create new header and footer definitions

Create definitions for headers and footers that don’t exist in the PDF plugin by default

So far, you’ve edited existing header definition templates in static-content.xsl. But there are a few header definitions that our PDF specifications call for that do not exist by default in static-content.xsl: TOC first, index first, TOC last, and index last. There are some footer definitions that also don’t exist by default: TOC first footer, index first footer, TOC last footer, and index last footer.

What do I mean by "do not exist by default"? I mean that if you open static-content.xsl and look at all the xsl:call-template statements at the beginning of the file, you won’t see anything like <xsl:call-template ...
