If you've ever written a book, created a design document, or, for that matter, researched a subject and wrote a paper on it, you are well aware that it's an iterative process. It seems as if no matter how many times you go over your work, new issues surface, flaws and errors are uncovered, and refinements are required. At some point, it becomes a process of diminishing returns. There wouldn't be too many publications out there if the standard were absolute perfection.
