Agriledger/AgUnity

The Agriledger platform from AgUnity (https://agunity.com) uses blockchain technology to create trust for small farmers in developing countries. They focus both on providing access to regional and international markets as well as capital for small farmers (who reportedly supply around 80% of the food consumed in their countries). The system is built so that it can run off a smartphone using a mobile app. The AgUnity app records all details of the farmer's crops and transactions, and it locks those details to the blockchain via the use of a QR code.
