To make things happen as smoothly as possible, we actually recommend a series of plans, or five types of transition plans:

  1. A Management Plan to define who will pay attention to day-to-day business, and who will be at least temporarily reassigned to lead, plan, and manage the change
  2. An Action Plan to identify transition activities, and then to sequence, time, and assign responsibility for the implementation of the design choices
  3. A Stakeholder Commitment Plan to determine who needs to be committed to the design, where those people are now, and where they need to be
  4. An Evaluation Plan to ensure that the design is doing what was intended and to watch that design drift doesn't set ...
